About the Program
Integrated Services Northwest (ISN) provides Intake and Service Coordination for adult developmental services.

The Adult Community Case Manager Program provides the following services:

  • Direct contact information regarding services and supports across the Kenora/Rainy River District.
  • Referrals and advocacy for individuals with intellectual disabilities to assist in developing greater independence and inclusion in the community.
  • Case management to ensure that individuals are provided with services they require in a coordinated, effective and efficient manner.

Eligibility Criteria for the Adult Community Case Manager Program
To be eligible for adult developmental services, a person must be at least 18 years of age and must satisfy one of the following diagnostic criteria:

  • A formal written diagnosis of Mental Retardation (mild, moderate or severe) and (may include) Pervasive Development Disorder.

OR

  • All of the following must be satisfied:
  1. IQ score equal to or <70
  2. Relevant assessments obtained that document the individual’s functioning is severely affected in two or more areas of adaptive functioning
  3. Age of onset is 18 years of age or less

The adult community case manager will determine the need for additional assessments and/or documentation to establish eligibility where it is unclear or ambivalent.

Cost
There are no costs for clients. This program is funded by the Ministry of Community and Social Services.

Location
The Adult Community Case Manager Program covers the entire Kenora/Rainy River District.
